Description
An Advanced Care Directive is a general term for a legal document that describes and instructs others about your medical care, in the event that you are unable to make your decisions known.Advanced Care Directives come in two forms: living wills for health care and durable power of attorney, or healthcare power of attorney. An Advanced Care Directives can only be made as long as a person has the capacity to make decisions and can fully understand the instructions they are taking.
